Individuals.  Individuals of the corporations shall be persons who:

a.         provide services to dental benefit plans or their enrollees, or the dental plan industry;

b.         subscribe to and support the corporation’s purposes; and

c.         are current in their payment of dues.

Associates.  Associates of the corporation shall be legal entities (or divisions or departments of legal entities) which:

a.         either

1.         provide to dental benefit plans or their enrollees or the dental benefit plan industry either:

A.         institutional services under contracts with dental benefit plans (e.g., hospitals, dental group practices, medical/dental IPA’s, medical groups and pharmacy programs), or

B.         non-medical professional services or dental or medical goods (e.g. accounting, actuarial, legal, managed care consulting firms; insurance agents, dental or medical supply vendors and manufacturers and pharmaceutical producers), or

C.        other related affiliated associations, or

2.         as part of a university conduct a program related to the health care field such as a university health service, a university dental benefit plan or an academic department of a university; and

b.         do not hold a state license as described in 2.1.1.(a)(1) above
